An official report says many warning signs were ignored before one of New Zealand's worst mining disasters. Twenty-nine miners died in November 2010, when the Pike River coal mine exploded. An investigation has found there were 21 warnings that the levels of methane gas were too high inside the mine premisis. The commission has now recommended that New Zealand revise its health and safety standards. Al Jazeera's Caroline Malone explains.

Breaking News Chinese coal mine roof collapse in Shaanxi kills 21 At least 21 miners died when a roof collapsed in a coal mine in northern China, officials say. Sixty-six miners were rescued after the accident on Saturday at the Lijiagou mine near the city of Shenmu in Shaanxi province. The cause of the collapse is under investigation, the official Xinhua news agency reported. Mining accidents in China are quite common despite efforts to improve safety. The Lijiagou mine is operated by the Baiji Mining Company, Xinhua said. No further details of the incident were available. Last October, 21 coal miners were killed when an underground rock fall blocked a shaft in eastern Shandong province. According to the latest figures from China's National Coal Mine Safety Administration, there were 375 deaths in coal mines in 2017, a fall of about 28% on the previous year. In a statement last January, the bureau said the "situation of coal mine safety production is still grim" despite improvements.

The train was travelling north from the town of Kamina in the southern province of Katanga to Mwene-Ditu in the diamond-mining province of Kasai-Oriental, AFP reported. According to Daily Times Nigeria, the accident happened near the city of Likasi. The goods train was also carrying passengers and was reportedly going too fast. When it came to a turn, the Daily Times Nigeria reported, the driver had to break suddenly, causing the train to overturn. Twelve of the carriages flipped off the track and at least 50 people were trapped inside. An investigation has now been set up to establish the cause of the accident, but according to witnesses quoted by AFP hundreds of passengers were both in and on the train's cars and many them had paid an illegal reduced fare to ride the train without a proper ticket. Congo's infrastructure is in tatters after decades of neglect and conflict, Reuters reported, making it hard for people to travel around the nation. Train accidents are common. More than 100 people were killed in a 2007 train accident in the province of Kasai Occidental. The World Bank is currently funding a refurbishment of the country's train network, which has received few upgrades since the country gained independence from Belgium in 1960, AFP reported. ------------------------------------------------------------------------------------------------ Welcome to TomoNews, where we animate the most entertaining news on the internets. Worker seriously injured after being hit by forklift and pallet at Barangaroo worksite in inner Sydney A man has been seriously injured after a pallet of scaffolding reportedly fell on him after he was hit by a forklift at a Barangaroo worksite in inner city Sydney. At 10:20am emergency services were called to the construction site on Sussex Street following reports of a workplace accident. The man, believed to be aged in his 40s, suffered multiple injuries after he was struck by a forklift carrying pallets of scaffolding. A pallet then fell off the vehicle and onto the worker. Rita Mallia from the Construction Forestry Mining and Energy Union (CFMEU) said the man was injured in a "double whammy". We're concerned that maybe the person who was driving the forklift truck was not a ticketed driver when in fact the ordinary driver was not here today. Rita Mallia, CFMEU "There was a forklift driver that has not seen a worker that was working in an open area, [he] has hit that worker and one of the scaffold pallets has fallen off the forklift and hit the gentleman," Ms Mallia said. "Apparently he's in a very serious condition but we're also very concerned about how this could occur and why it was that a forklift driver was not able to see a worker in his path. "We're concerned that maybe the person who was driving the forklift truck was not a ticketed driver when in fact the ordinary driver was not here today." The man was taken to Royal North Shore Hospital by ambulance in a serious condition with injuries to his legs and pelvis. Ms Mallia said the union had some questions about what systems failed at the site to result in the accident. "We're concerned about how the lift was loaded, the security of the load obviously, why it is that vision was impaired and is it the case that this person was not ticketed to operate this machinery in the first place," she said. "This site has had issues since its inception. "I think over time things have gotten better, but it is a very big site, a very complicated site and we'd expect the highest level of standard from a builder like Lend Lease." Police and WorkCover are investigating the incident. In January last year, a man fell to his death at the Barangaroo construction site while working on scaffolding. The man was doing general construction work at the time and fell about 30 metres and died at the scene. Meanwhile in March last year, a fire at the site sparked by a welding accident forced thousands of evacuations and traffic chaos in Sydney. http://www.abc.net.au/news/2015-08-08/man-seriously-injured-in-barangaroo-construction-accident/6682746 Clip from Nine News
